Assessment and Intake: Important Information
This is a 2 hour in-person appointment. This is your opportunity to learn about the program, ask your questions, and provide us with information about your situation and how you believe we can help and support you.
Please bring all your paperwork to this appointment including 2 current pay stubs, any court documents (restraining orders, sentencing judgments), referral paperwork, paperwork regarding monthly expenses, and one source of identification.
Payment for assessment is due before you arrive for your appointment and is payable when you book your appointment on our website. Assessment and intake is a flat rate of $130 (not including taxes). This payment is non-refundable unless you cancel or reschedule 24 hours or more before your appointment date and time. Group session fees are based upon consultation.
Please arrive on-time as arriving past the grace period of 10 minutes will be considered a "no show" to the appointment and fees will not be refunded.
